Emergency Medical Authorization & Information

In the event I cannot be reached to make arrangements for emergency medical care, I authorize The Mays School to call 911 and request an ambulance to transport my child to the medical facility recommended by EMTs and/or transport my child to the closest/most appropriate medical facility, and to secure any and all necessary emergency medical care for my child.

Acknowledgement of Policies

I understand that it is my responsibility, as the parent or guardian, to keep all of my family’s information up to date at MAYS, including information about phone numbers, emergency contacts, addresses, authorized pick-up persons, allergies, illnesses, special needs, immunization records, health records, and all other basic information.

I understand that unless legal documentation is submitted to the contrary, we assume that a child’s parents share equal rights to drop off or pick up.

I understand that pictures of children enrolled at MAYS will be displayed in classrooms, hallways, newsletters, and on the MAYS website and social media pages.

I understand that MAYS provides sunscreen, insect repellant, and wet wipes in the classroom, and on the playground, unless I opt out by sending an email to info@mays.school. Please see the Parent Handbook for the specific brands used.

I understand that all children participate in outdoor play each day (including rainy, snowy, hot, and cold days). Students spend an average of three hours outside each day. I understand that if my child is not able to participate in outdoor activities, then they need to stay home for the day.

I have read and understand the tuition and withdrawal policies at The Mays School.

I GIVE consent for my child to be transported to (using a MAYS vehicle) and participate in scheduled field trips and off-campus extracurricular classes once they are three years of age or older. I understand that my child will be supervised by a MAYS staff member. Additional permission slips will be sent home one week or more before the trip and will need to be signed before any child leaves campus.

I understand that my child must wear a MAYS-branded top while attending Summer @ MAYS. One MAYS shirt will be provided with enrollment, and additional apparel can be purchased in the front office.

I understand that MAYS offers tiered refunds for early withdrawal:

--A full refund will be given if you withdraw before the early notice deadline of April 1, 2027.

--A 50% refund will be given if you withdraw before the next deadline of June 1, 2027. Registration can also be moved to another camp session at MAYS for no additional fee.

--No refund is given if you withdraw later than June 1, 2027.
