I give permission for my child to participate in water activities that include swimming pools and for my child to be supervised by Wolfies Swim School’s staff during swim lessons and while at their location (5230 Cedar St. Bellaire, TX 77401).

I give consent for my child to be transported and supervised by MAYS staff for emergency care and field trips including to Wolfies Swim School located at 5230 Cedar St. Bellaire, TX each week for swim lessons.

I understand that, for each swim lesson, I am required to send my child to school with the following items: MAYS swim rash guard shirt, swimsuit bottoms, MAYS cover-up, approved water shoes, Happy Nappy Duo swim diaper (if needed), and an extra change of clothes. Please make sure all items are labeled with your child’s full name using waterproof labels.

I understand that swim classes run 9/15/25 through 6/12/26 and that my child will receive a total of 35 swim lessons. Tuition is $1225 ($35 per lesson), plus a non-refundable $100 supply fee. Tuition is due September 1^st, 2025, or paid in three equal increments (September 1^st, December 1^st, and March 1^st). You are registering for the full school year. Discounts and refunds are not available.

I understand that I can purchase additional swim supplies in the front office, but my swimmer cannot attend lessons without the proper swim attire.

I understand that Wolfies offers one make-up class per month. MAYS must be notified via email (info@mays.school) by 10:00 AM on the day of the class, or sooner, to be eligible for a make-up class. Make-up classes will be available through the Wolfies app and can be scheduled at a convenient time for your family. MAYS will not be able to provide transportation to make-up classes, but evening and weekend times are available, and make-up classes are available throughout the 2025-2026 school year.

All children attending swim lessons/swim team are assumed to be unable to swim competently, as defined by the American Red Cross. All students will be supervised by Wolfies staff with this knowledge in mind.

If your child is too sick to attend school, then they are too sick to attend swim lessons. MAYS’ current illness policies are in effect for swim lessons. Due to the nature of swim lessons, Wolfies has an illness policy that is different than ours: students are required to remain out of swim lessons for two full weeks following any episode of diarrhea (counting from the last day of symptoms).

In the event I cannot be reached to make arrangements for emergency medical care, I authorize The Mays School and Wolfies Swim School to call 911 and request an ambulance to transport my child to the medical facility recommended by EMTs. I authorize The Mays School to transport my child to the closest/most appropriate medical facility. I authorize The Mays School to secure any and all necessary emergency medical care for my child. I authorize my child’s pediatrician to release any necessary medical information to The Mays School in the event of an emergency.

WOLFIES SWIM SCHOOL WAIVER

I, acting on behalf of myself, my family members, all other members of my household, all guests or persons employed by me accompanying the above persons, as well as in the capacity as the legal guardian of this child and/or children named, (collectively the "Participants"), being of lawful age, hereby release and discharge Cindy Wolf Meador, Wolfies Swim School LLC, The Mays School and its owners, officers, directors, employees, agents, successors, legal representatives and assigns (collectively the "School") from all claims, demands, actions, judgments and executions of any kind, nature and description, including without limitation those arising with respect to swimming lessons or in any way related to any swimming programs or related activities conducted by or associated with the School, its facilities and/or any facilities used by the School (the "Programs"), whether arising out of past, present, or subsequent events, whether known or unknown, without regard to the cause or causes thereof or the negligence of the School, from and against any and all liability, loss, damage, or expense, including reasonable attorney's fees, that the School may suffer or incur as a result of any claims, demands, costs, litigation, injury to persons or property, death or judgment against the School of every kind and character by, for, through or on behalf of the Participants, without limitation and without regard to the cause or causes thereof or the negligence of any party or parties, including the School, with respect to the Programs. The Participants agree to maintain in force adequate insurance coverage, including, without limitation, property, casualty, liability, and other insurance, sufficient to cover all of their obligations under this Agreement. I hereby represent and warrant that I have legal authority to act on the behalf of the Participants and to legally bind the Participants to the terms of this Agreement. By signing below, I hereby acknowledge that I have noted on the back of this agreement any and all medical history or other information of which the School should be aware that would or could affect participation of any of the Participants in the Programs. The Participants hereby authorize the School to treat any Participants in any medical emergency arising during or incident to the Programs.

I have received and understand Wolfies Swim School and The Mays School’s policies. I understand that Wolfies Swim School LLC, The Mays School, and its staff members cannot be held liable for injuries at 5230 Cedar, Bellaire TX 77401 or while transporting to the location. Knowing that Wolfies Swim School and The Mays School will do its best for the safety of its clients, I assume all responsibility and waive any claim for compensation for accidental injury while at 5230 Cedar Bellaire TX 77401 and hereby agree to indemnify to hold harmless Wolfies Swim School LLC and its staff against any and all claims which may arise from any injury while participating in the swimming lessons. I HAVE CAREFULLY READ THE ABOVE LIABILITY RELEASE AND SIGNED IT WITH FULL KNOWLEDGE OF ITS CONTENTS AND SIGNIFICANCE.
